Here the payout account comes before the account exists, not at the first deposit. The practical effect: your withdrawals go to the account you link here, and the name on it has to match the name in the form.
Changing that account afterwards means a conversation with support and a fresh name check, so link the wallet or bank you will still use a year from now. If you share an eSewa account with a family member, open your own first.
Linking someone else's account "just for now" can leave a payout on hold, because support has no way to confirm the money is yours.
EVO33 referral code: what the field does
The last box on the form is labelled for an affiliate or referral code. It links your account to whoever invited you. EVO33 lists a referral promotion for existing players who bring friends, with the rules on its promotions page.

What does the EVO33 registration form ask for?

Does EVO33 send an SMS code?
Yes. The form sends a one-time code to your mobile number, and you type it into the OTP box before you submit.
